  • Key Clinical Development Updates
    • Telix secured FDA alignment via a successful Type B meeting confirming Part 1 safety suffices to initiate U.S. Part 2 of the ProstACT Global Phase 3 for TLX591‑Tx (177Lu‑rosopatamab tetraxetan) in mCRPC, with agreement on the Part 2 protocol, statistical analysis plan, and safety monitoring, enabling an IND amendment submission pending FDA review.
    • ORIC initiated Himalayas‑1, a global Phase 3 registrational trial of the PRC2/EED inhibitor rinzimetostat plus darolutamide in mCRPC previously treated with abiraterone following End‑of‑Phase 1 interactions that finalized the protocol, with Bayer supplying darolutamide under a clinical collaboration and supply agreement.
  • Key Conference Updates
    • July meetings highlighted momentum toward alpha‑emitting PSMA radiotherapeutics and operational workflows for Pluvicto—including toxicity monitoring and imaging‑led patient selection—pointing to deeper responses and safer delivery beyond current beta emitters.
    • Conference and ASCO coverage emphasized ABBV‑969, a dual PSMA/STEAP1 Topo‑1i ADC showing promising activity and manageable safety in heavily pretreated mCRPC, and the Phase 3 evaluation of pasritamig (KLK2‑targeted T‑cell engager) with docetaxel versus docetaxel alone toward registrational intent.
  • Emerging Themes & Macro-Trends
    • Late‑July CMS 2027 OPPS and PFS proposed rule summaries signaled potential impacts on radioligand therapy reimbursement pathways, and the ASCO Living Guideline v2026.1.4 (July 20) refined sequencing frameworks anchoring real‑world adoption.
    • Strategic imperatives coalesced around investing in PSMA‑alpha platforms, ctDNA/PET analytics, and dual‑antigen ADCs while scaling theranostics operations with supply and quality‑assurance readiness to secure first‑mover advantage.
FDA Approvals/Withdrawals
FDA Approvals/Withdrawals
  • On July 31, 2026, the FDA approved lutetium Lu 177 vipivotide tetraxetan (Pluvicto, Novartis Pharmaceuticals Corporation) in combination with androgen receptor pathway inhibitor (ARPI) therapy for adults with prostate-specific membrane antigen (PSMA)-positive metastatic androgen pathway modulation-naïve or -sensitive (mAPMN/S) prostate cancer. Exploratory Time-Dependent Patterns in Estimated Treatment Effect of Talazoparib Plus Enzalutamide in HRR Non-Deficient or Unknown Metastatic Castration-Resistant Prostate Cancer: A Post Hoc Analysis of the TALAPRO-2 Trial. This post hoc analysis of the TALAPRO-2 trial reconstructed patient data to explore time-dependent treatment effects of talazoparib plus enzalutamide in patients with mCRPC without HRR deficiencies. While not statistically significant, the findings are hypothesis-generating, suggesting potential for delayed overall survival benefits and early radiographic progression-free survival benefits in this specific subgroup. ()
Matching-adjusted indirect comparison of enzalutamide versus apalutamide with androgen-deprivation therapy in patients with metastatic hormone-sensitive prostate cancer. This matching-adjusted indirect comparison of the ARCHES (enzalutamide) and TITAN (apalutamide) trials suggests broadly similar efficacy between enzalutamide and apalutamide for patients with metastatic hormone-sensitive prostate cancer (mHSPC). The analysis found comparable outcomes for overall survival and time to PSA progression, with no significant differences in radiographic progression-free survival after sensitivity analyses. (, )

Theranostic Potential of 177Lu-TLX591 with Best Standard-of-Care and 68Ga-PSMA-11 PET for Patients with Metastatic Castration-Resistant Prostate Cancer: Results from the Phase 1 ProstACT SELECT Trial. The ProstACT SELECT trial demonstrated that the radio antibody-drug conjugate 177Lu-TLX591, when combined with standard of care, shows a manageable safety profile and durable tumor retention in mCRPC patients selected via 68Ga-PSMA-11 PET. Notably, this agent resulted in low salivary gland radiation exposure, a key differentiator from small molecule radioligand therapies, supporting its continued development. ()
Randomized phase II trial of nivolumab and ipilimumab with or without stereotactic body radiation therapy in patients with metastatic castration-resistant prostate cancer: the CheckPRO, CA209-8TY trial. In the CheckPRO trial for heavily pretreated mCRPC, the addition of stereotactic body radiation therapy (SBRT) to the nivolumab and ipilimumab combination did not improve PSA response rates or overall survival. While the combination of nivolumab and ipilimumab alone showed activity in a fraction of patients, these findings do not support the addition of SBRT and underscore the need for predictive biomarkers for immunotherapy in this setting. ()
Durable responses to triplet immunotherapy targeting TGF-β, PD-L1, and tumor antigen, with an IL-15 receptor superagonist in mismatch repair proficient castration-resistant prostate cancer. A novel triplet immunotherapy, combining a vaccine (BN-Brachyury), a dual PD-L1/TGF-β inhibitor (bintrafusp alfa), and an IL-15 superagonist (nogapendekin-alfa inbakicept), achieved sustained PSA declines in 29% of patients with mismatch repair proficient CRPC. This multi-pronged strategy, which activates both NK and CD8+ T cells, represents a promising approach for this immunologically "cold" tumor type. ()
