New Publications Related to Drugs in Clinical Trial Development
Metastasis-directed Therapy With or Without Pembrolizumab for Oligometastatic Clear Cell Renal Cell Carcinoma: Pooled Analysis of Two Prospective Single-arm Phase 2 Trials. A pooled analysis of two phase 2 trials suggests that adding maintenance pembrolizumab to radiotherapy-based metastasis-directed therapy (MDT) improves progression-free survival compared to MDT alone in patients with oligometastatic clear cell renal cell carcinoma. The combination therapy also induced favorable immunomodulatory changes, including the systemic induction of activated CD8+ T cells, supporting the rationale for the ongoing randomized ASTROs trial. ()
Elraglusib and chemotherapy in metastatic pancreatic ductal adenocarcinoma: a randomized controlled phase 2 trial. The addition of the GSK-3β inhibitor elraglusib to gemcitabine and nab-paclitaxel significantly improved median overall survival and 1-year survival rates in patients with previously untreated metastatic pancreatic ductal adenocarcinoma. These findings support the clinical activity of this combination as a first-line treatment and provide a biological rationale for advancing to phase 3 trials. ()
Phase II Trial of Ixazomib Combined with Gemcitabine and Doxorubicin in Patients with SMARCB1-Deficient Renal Medullary Carcinoma. In a phase II trial for SMARCB1-deficient renal medullary carcinoma, the addition of the proteasome inhibitor ixazomib to gemcitabine and doxorubicin modestly increased the objective response rate but failed to extend the 28-week disease-control rate, crossing the predefined futility boundary. Multi-omic correlative analyses revealed that immune-inflamed tumors correlated with response, while stromal-myeloid niches and specific plastic epithelial states were associated with resistance, providing critical insights for future mechanism-directed trials. ()
Fecal microbiota transplantation plus immunotherapy in metastatic renal cell carcinoma: the phase 1 PERFORM trial. The phase 1 PERFORM trial demonstrated that healthy donor fecal microbiota transplantation (FMT) combined with first-line immune checkpoint inhibitors is safe and yields a 50% objective response rate in patients with metastatic renal cell carcinoma. Durable engraftment of anti-inflammatory taxa correlated with reduced toxicity and improved response, highlighting the potential of functional microbiome modulation to optimize immunotherapy outcomes. ()
CALYPSO: Final Results of Savolitinib and Durvalumab Combination in Metastatic Papillary Renal Cancer. Final results from the CALYPSO study confirm that the combination of the MET inhibitor savolitinib and the anti-PD-L1 antibody durvalumab provides clinically meaningful overall survival benefits, particularly in patients with MET-driven metastatic papillary renal cancer. Furthermore, exploratory analyses suggest that circulating tumor DNA clearance and variant allele frequency reduction may serve as useful predictive biomarkers for treatment response. ()
Cabozantinib in patients with non-locally pretreated brain metastases from renal cell carcinoma: Results of the multicenter phase II trial CABRAMET. The phase II CABRAMET trial demonstrated that cabozantinib exhibits significant and prolonged efficacy in patients with non-locally pretreated brain metastases from renal cell carcinoma, achieving a 61.5% objective response rate in the brain. This study highlights cabozantinib as a highly active and tolerable systemic treatment option for this challenging patient population, particularly in the first-line setting. ()
KIM-1 in Advanced Papillary and Clear Cell Renal Cell Carcinoma. An exploratory analysis from the CALYPSO trial revealed that Kidney Injury Molecule-1 (KIM-1) levels are significantly higher in advanced papillary renal cell carcinoma (pRCC) compared to clear cell RCC (ccRCC) and decrease with systemic therapy in both subtypes. In ccRCC, high baseline KIM-1 and on-treatment increases were associated with worse survival outcomes, supporting its potential utility as a prognostic and predictive biomarker in advanced disease. ()
Fecal microbiota transplantation plus pembrolizumab and axitinib in metastatic renal cell carcinoma: the randomized phase 2 TACITO trial. In the randomized phase 2 TACITO trial, the addition of healthy donor fecal microbiota transplantation (FMT) to first-line pembrolizumab and axitinib significantly prolonged median progression-free survival compared to placebo FMT in patients with metastatic renal cell carcinoma. Microbiome analysis indicated that the acquisition or loss of specific bacterial strains, rather than total engraftment, was associated with improved clinical outcomes, supporting the potential efficacy of selected donor FMT to enhance immunotherapy.
