Novel pembrolizumab-based treatments as first-line therapy in advanced clear-cell renal cell carcinoma: substudy 03A of the open-label, umbrella platform, phase I/II KEYMAKER-U03 trial. This Phase 1/2 substudy evaluated novel pembrolizumab-based combinations in treatment-naive advanced clear-cell renal cell carcinoma, reporting a 77.5% objective response rate and a median progression-free survival of 31.8 months for the triplet of pembrolizumab, lenvatinib, and belzutifan. While the efficacy was robust, the triplet performed similarly to the doublet control arm, and the specific contribution of belzutifan in this setting is being further defined in the ongoing Phase 3 LITESPARK-012 study. Cytoreductive nephrectomy in the era of immune-checkpoint inhibitors: back to the future? This narrative review analyzed 12 retrospective studies regarding the role of cytoreductive nephrectomy (CN) in metastatic renal cell carcinoma treated with immune-checkpoint inhibitors, noting a survival benefit associated with CN in half of the studies, particularly in the upfront setting. The authors conclude that while retrospective signals are positive, definitive practice changes regarding the timing and necessity of CN await readouts from ongoing randomized trials such as PROBE (SWOG-1931). Efficacy of the ATR inhibitor ceralasertib in patients with ARID1A-deficient gynecologic and other solid tumor malignancies. In this Phase 2 basket trial targeting ARID1A-deficient solid tumors, the ATR inhibitor ceralasertib demonstrated a 14% objective response rate, with all confirmed responses restricted to gynecologic malignancies. Although the study enrolled various solid tumors, the lack of responses outside of endometrial and ovarian carcinomas suggests that ceralasertib monotherapy may have limited activity in other ARID1A-deficient solid tumor types.
